Introduction

The LGBTQ population has often been marginalized due to provider discomfort or patients’ fears of disclosing their sexual orientations or gender identities.  Family Medicine Physicians are in the best setting to care for this population with our ability to form long-term relationships with our patients.  Seattle is one of those great cities where we have a multitude of services for our LGBTQ patients.  For those interested in expanding their knowledge in LGBTQ healthcare, Swedish First Hill Family Medicine Residency Program is proud to offer our LGBTQ Area of Concentration.  Please contact Kevin Wang to discuss your schedule for your area of concentration.

Goals & Objectives

This area of concentration will provide you with additional training to gain further expertise in the care of LGBTQ patients.  This one or two year longitudinal experience will provide you more opportunities to partner with Seattle's (and Washington's) LGBTQ community through advocacy, community service and additional medical training.  Some of these experiences are listed below:

  • Additional clinic experience working in LGBTQ clinics:

    • Capitol Hill Medical Group

    • Polyclinic (Dr. Kevin Hatfield who cares for Seattle's largest population of trans* adolescents)

    • Seattle Children's Gender Clinic

    • Drs. Tony Mangubat and Javad Sajan who provide top surgeries for our trans* & GNC patients

  • Work with volunteer organizations to provide additional support for our LGBTQ population:

    • Camp Ten Trees - Youth LGBTQ Camp

    • Seattle Gay News - Write an article discussing important health topics related to the LGBTQ population

    • Gay City - Gay Men's Wellness Clinic partnered with King County Public Health

    • Swedish LGBTQ Employee Meetup Group

    • Ingersoll Gender Center - Work with the provider consult group and with the peer support group for trans* people

  • Gain leadership skills to advocate for the LGBTQ community:

    • Attend the AAFP National Conference of Constituency Leaders to network with LGBT leaders in our academy to help form policy that affect our LGBT patients and providers

    • Work with local advocacy groups such as Legal Voice to help advocate for our LGBT community

    • Work with Swedish's LGBTQ Affinity Group to help guide Swedish towards creating a more inclusive environment for our patients and employees

    • Attend the GLMA Annual Conference

  • Provide education for our local health care providers:

    • Attend Swedish's annual LGBTQ CME

    • Give a regional presentation at the annual CME

Rotation Milestones

  1. Patient care

    • PC-1, PC-2 & PC-3: Learn to provide patient-centered and quality care by partnering with patients to counsel, inform, educate and support our LGBTQ patient population in areas of healthcare maintenance and other LGBTQ-related health needs

    • PC-5: Gain competence in performing appropriate counseling and prescribing of hormone therapy to meet the needs of our trans patient population

  2. Medical knowledge

    • MK-1 & MK-2: Demonstrate appropriate knowledge about LGBTQ healthcare relative to current level of training

  3. Systems-based practice

    • SBP-2: Emphasize patient safety following appropriate clinical guidelines and protocols

    • SBP-3: Advocating for individual and community health by partnering and working with community programs

    • SBP-4: Continue building skills to work with the healthcare team

  4. Practice-based learning & improvement

    • PBL-1 & PBL-2: Engage in self-directed learning and demonstrate abilities to acquire new skills in LGBTQ healthcare

  5. Professionalism

    • PROF-2 & PROF-3: Demonstrate compassion/respect/empathy towards patients and presents in a respectful & professional manner while attending all duties/responsibilities

  6. Communication

    • C-1, C-2 & C-3: Develop meaningful, therapeutic relationships with patients while communicating effectively with patients and the healthcare team

Application Requirements

Any resident in the Swedish First Hill Family Medicine Residency Program can participate in the LGBTQ Area of Concentration so long as they meet these criteria:

  • Must be in good standing in the residency program meeting all core competency requirements.

  • Must be on track to meet the required number of outpatient clinic visits, OB continuity deliveries and other ACGME requirements.

  • Must obtain approval from your advisor to participate in this Area of Concentration.

Requirements for Completion of the LGBTQ Area of Concentration

To successfully complete the LGBTQ AOC, residents must complete 1 LGBTQ elective during your R2 or R3 year in addition to attaining a total of 4 points from the following list:

  • Present an LGBTQ health talk (1 point for each lecture)

  • Apply to the GLMA Scholarship to attend their annual conference along with presenting the information to the residency program (2 points)

  • Additional LGBTQ-related elective along with presenting the information to the residency program (3 points)

  • Community Medicine and Advocacy work through the AAFP, WAFP or other organizations (2 points)

  • Longitudinal community service with Gay City at the immunization clinic (2 points)

  • Additional LGBTQ elective (2 points)

  • Present an LGBTQ-related topic at the Annual Swedish LGBTQ CME, AAFP National Conference for Residents and Medical Students or at the WAFP Medical Student/Resident Retreat (2 points)

  • HIV Training Certificate (3 points)

It is also encouraged to continue your work with our specialists to continue getting additional exposure and training such as continued work at the Capitol Hill Medical Group, Ingersoll Gender Center, Cedar River Clinic's LGBTQ Clinic, Seattle Children's Hospital's Gender Clinic and the Polyclinic.
